Why did it happen?
How did you ensure it wouldn't happen again?
Did you truly forgive him?
Has he forgiven himself?
Are you sure there's not an OW now?

It sounds like you have a number of things you can start working on:

- anger
- controlling
- insecure
- taking H for granted
- anxiety/depression
