Years ago I broke down and bought the d*mn wrench (pipe wrench) -- because there's no real substitute and gives the proper leverage to make the task oh so much easier.
Once you get it off I suggest using some silicone tape -- especially for metal-on-metal connections. It makes later removal of the showerhead much easier.
