On the therapist, you might look for someone like my IC (MC) who is subsidized by a church to keep his rates very low. I pay him out-of-pocket as a result, which is very reasonable considering. (And I am not even a member of the particular church, BTW.)
Like I hear others say, a therapist is certainly going to be a lot less expensive than a D. So please keep trying.
