It seems he is very up and down. Sounds like my H. Is so hard to try and figure out where the heck they are coming from sometimes. No, don't blame yourself for not seeing this. this is not your fault. You love him, of course you miss him, but you don't miss the person he's become. You need to concentrate on you and the kids. Do things for you and keep your distance.
