I just got a nasty letter from her demanding an increase in child support. She wants $500 more a month then I already give her when I only make $200 more a month then she does. I told her I'd see her in court.

Then you call your insurance company and explain to them the need to get off their asses RIGHT NOW because they are holding up your therapy and that is UNACCEPTABLE.
You can also tell that receptionist - hell, I'd love to do it for you, but you can tell her uppity ass that while it's great life's all hunky flippin dory for her, some people need help with problems larger than chipping a nail and she needs to either make you an appointment now or have the doctor give you an immediate referral.
