Thanks Ellie, We pretty much figured it was the sperm granuloma. What exactly is involved in the "vent" surgery. He's taken anti-inflammatories and antibiotics several times to no avail. I'm guessing surgery may be required. He's not real keen on the idea since his vasectomy has seemed to give him so much trouble, but he's also said he's tired of being in pain and will do whatever it takes to fix it.
